This policy explains how Core Health Rehabilitation ("we") handles information you share through this website. It covers the website only. Information you share with us in person as a patient is handled separately, under the privacy rules that apply to health care providers.

What we collect

• If you join our opening list: your email address.
• If you request an appointment: your name, phone number and email address, whether you are looking for physical therapy, chiropractic or both, your preferred appointment time, and anything you choose to write in the note field.
• Technical details sent with a form, such as the page you were on, your browser type, your IP address and any campaign tags in the page address. We use these to keep the forms working and free of spam.

Please do not include medical history or other health details in a website form. We take your full history in person, at your first visit.

How we use it

• Opening list: to send a one-time confirmation that you joined, then to email you once when we open for appointments. We do not add you to any other mailing.
• Appointment requests: to call or email you about the appointment you asked for.

We do not sell your information, and we do not share it with anyone for their own marketing.

Where it is kept

Form submissions are stored in a secured database run by our website host, Cloudflare. Our staff are notified of each submission by email, and emails are sent through Amazon Web Services (Amazon SES). Cloudflare Turnstile checks that each submission comes from a person rather than an automated program, and processes technical information about your browser to do so.

How long we keep it

We keep opening-list addresses only until we have sent the opening announcement. We keep appointment requests only as long as we need them to arrange your visit and to meet our record-keeping obligations.

Cookies and analytics

This website does not use advertising cookies. If we add website analytics, we will update this policy to say what is measured and how to opt out.

Your choices

You can ask us to show you, correct, or delete the details you sent through this website. Call us on732 534 3866.

Children

This website is not directed at children. A parent or guardian should request appointments for anyone under 18.

Changes

If we change this policy, we will update it here and change the date at the top.
